Add Program Personnel

To open a new program, you must submit all completed forms for each personnel type listed below.

Once you have an approved program, you must complete and submit forms for approval before adding any of the personnel types listed below.

** Please note that any orientation scheduled for a state holiday will be cancelled for that month.

To apply to be an Instructor, please complete the following application and email it to WABONNursingAssistantsED@doh.wa.gov

Notification of Program Owner and Owner Declaration

Add a New Program Owner

When a nursing assistant training program adds a new program owner(s), the new owner(s) must complete and submit to WABONNursingAssistantsED@doh.wa.gov:

WABON staff and the Nursing Assistant Program Approval Panel (NAPAP) will review your submissions and respond promptly.
