Board of Nursing, Licensing Discipline Update

Olympia

The Washington State Board of Nursing (WABON) has taken disciplinary action or withdrawn charges against the following healthcare providers in our state.

WABON is committed to protecting public health and safety by regulating the competency and quality of all Licensed Practical Nurses (LPNs), Registered Nurses (RNs), Advanced Registered Nurse Practitioners (ARNPs), and Nurse Technicians (NTECs).

Asotin County

In July 2025 the Board of Nursing released Alexis Danielle Carr (RN60624369) from the terms and conditions on her registered nurse license.

Benton County

In June 2025 the Board of Nursing granted Surrender Phoenix (RN61635159) a license to practice as a registered nurse with conditions on the license.  Phoenix must continue to comply with a monitoring program.

Chelan County

In July 2025 the Board of Nursing granted Madison Hope Colpitts (LP61649975) a license to practice as a licensed practical nurse with conditions on the license. Colpitts must continue to comply with a monitoring program. In April 2022 an order for deferred prosecution was entered following a charge of driving under the influence.

In May 2025 the Board of Nursing indefinitely suspended the licensed practical nurse license of Jessica N. McDonald (LP00055112). Prior to petitioning for reinstatement, McDonald must contact a monitoring program and, if required, enroll in the monitoring program. McDonald failed to comply with a 2024 stipulation by failing to sign a monitoring contract and enroll in a monitoring program.

Clark County

In May 2025 the Board of Nursing reinstated the registered nurse and advanced registered nurse practitioner licenses of Peter Karera Ndungi (RN60938193, AP60938195-NP) subject to conditions. Ndungi must continue to comply with a monitoring program.

Cowlitz County 

In May 2025 the Board of Nursing withdrew the statement of charges on the registered nurse license of Brianna Janel Williams (RN61065823).

Grant County

In June 2025 the Board of Nursing indefinitely suspended the registered nurse license of Korina E. Casillas (RN60023215). Prior to petitioning for reinstatement, Casillas must contact a monitoring program and, if required, enroll in the monitoring program. Casillas must also complete a minimum of ten hours of continuing education in the area of documentation. In January 2023, Casillas pulled narcotics for several patients but failed to document administration or wastage of the narcotics. Casillas failed to cooperate with the investigation by not furnishing a written response to the allegations.

King County 

In August 2024 the Board of Nursing charged registered nurse Alezandria Curry (RN61389197) with unprofessional conduct. Charges state that in October 2023 after a verbal altercation with a patient, Curry identified the patient to a friend who then confronted the patient.  

In May 2025 the Board of Nursing and Mariya Igor Fair (RN60320185, AP60324589-CRNA) entered an agreed order on her registered nurse and advanced registered nurse practitioner licenses. Fair must contact a monitoring program and, if required, enroll in the monitoring program. Between approximately November 2023 and February 1, 2024, Fair diverted narcotics for her own use.  

In April 2025 the Board of Nursing and Alarik Isaac Fryling (RN60334594) entered an agreed order on his registered nurse license. Fryling must contact a monitoring program and, if required, enroll in the monitoring program. In July 2024, the California Board of Registered Nursing accepted a stipulated surrender of Fryling’s registered nurse license. Fryling violated terms of a prior board order.  

In May 2025 the Board of Nursing charged registered nurse Tara Jean Kelly (RN60375903) with unprofessional conduct. Charges state that in September and October 2023, Kelly engaged in practices resulting in controlled substance discrepancies. Kelly also failed to cooperate with the investigation by not furnishing a written response to the allegations.  

In July 2025 the Board of Nursing charged registered nurse Joseph Lee Lariza (RN61133543) with unprofessional conduct. Charges state that Lariza failed to comply with a 2024 stipulation by failing to sign a monitoring contract.  

In May 2025 the Board of Nursing charged registered nurse Jonathan Caleb Magness (RN61384120) with unprofessional conduct. Charges state that between approximately June 12, 2023 and July 29, 2023, Magness engaged in practices resulting in controlled substance discrepancies. Magness also failed to cooperate with the investigation by not furnishing a written response to the allegations.  

In May 2025 the Board of Nursing charged and immediately suspended the registered nurse license of Stephany Olander (RN61446185) pending further disciplinary action. In March 2025, the California Board of Registered Nursing revoked Olander’s registered nurse credential.

Okanogan County 

In July 2025 the Board of Nursing reinstated the registered nurse license of Tanna N. Vigue-Dry (RN00155709) subject to probation. Vigue-Dry may not practice as a nurse except as part of a pre-approved refresher course. Vigue-Dry must also contact a monitoring program and, if required, enroll in the monitoring program.

Pierce County 

In April 2025 the Board of Nursing indefinitely suspended the licensed practical nurse license of Brien Edwin Applonie (LP00041235). Applonie failed to comply with a 2023 board order by failing to provide proof of completion of an approved refresher course.

In June 2025 the Board of Nursing indefinitely suspended the registered nurse license of Peter Meric Atanasoff (RN60844697). Atanasoff failed to comply with a 2024 board order by failing to provide proof of completion of required course work and by failing to have a mental health evaluator submit an evaluation report.

In April 2025 the Board of Nursing indefinitely suspended the licensed practical nurse license of Caroline Brooks (LP61241106). Prior to petitioning for reinstatement, Brooks must contact a monitoring program and, if required, enroll in the monitoring program. Between approximately October 29, 2023 and January 2024, Brooks made documentation and/or medication errors.

In March 2025 the Board of Nursing indefinitely suspended the registered nurse license of Amy Jo Bruner (RN60869362). Prior to petitioning for reinstatement, Bruner must contact a monitoring program and, if required, enroll in the monitoring program. In 2022, Bruner was observed to demonstrate signs of intoxication at her place of employment. Bruner failed to cooperate with the investigation by not furnishing a written response to the allegations.

In July 2025 the Board of Nursing indefinitely suspended the licensed practical nurse license of Bakari Shaban Gakere (LP60344117). Prior to petitioning for reinstatement, Gakere must obtain a mental health evaluation and complete pre-approved continuing education. In February 2023, Gakere engaged in a physical altercation with a patient.

In August 2025 the Board of Nursing charged registered nurse and advanced registered nurse practitioner David Anthony Gonzalez (RN60977137, AP60977138-NP) with unprofessional conduct. Charges state that Gonzalez failed to comply with a 2025 stipulation by failing to enter a monitoring program.

In June 2025 the Board of Nursing permanently revoked the licensed practical nurse license of Shawntea G. Hamilton (LP61033790). In May 2024, Hamilton was convicted of second-degree manslaughter-domestic violence.  

In May 2025 the Board of Nursing charged registered nurse Luis R. Ibarra (RN00144561) with unprofessional conduct. Charges state that in December 2022, Ibarra was convicted of rape in the second degree of a patient or client.

In May 2025 the Board of Nursing reinstated the licensed practical nurse license of Roger S. Miller (LP00056350) subject to probation. Miller may not practice as a nurse except as part of a pre-approved refresher course and must continue to comply with a monitoring program.  

In June 2025 the Board of Nursing suspended the registered nurse license of Michael Killen Sumner (RN60820063). Sumner may not petition for reinstatement for a period of at least five years. Prior to petitioning for reinstatement, Sumner must contact a monitoring program and, if required, enter the monitoring program. In September 2024, Sumner was convicted of vehicular homicide and felony DUI in King County Superior Court.

In June 2025 the Board of Nursing indefinitely suspended the registered nurse license of Melissa Ann Teague (RN61345213). Prior to reinstatement, Teague must contact a monitoring program and, if required, enroll in the monitoring program. In March 2024, the Oklahoma Board of Nursing entered an order placing terms and conditions on Teague’s license. In April 2024, the Oklahoma Board suspended Teague’s license for failure to comply with the order.

Skagit County 

In July 2025 the Board of Nursing released Katherine Ann Owens (RN60235924) from the terms and conditions on her registered nurse license.

Snohomish County 

In May 2025 the Board of Nursing granted Alisa Rud (RN61640050) a license to practice as a registered nurse with conditions on the license.  Rud must continue to comply with a monitoring program. In November 2019, Rud was convicted of negligent driving. In March 2022, Rud was charged with driving under the influence and entered into a deferred prosecution agreement in September 2024.

In July 2025 the Board of Nursing released Cynthia Dawn Shaw (RN60286419) from the terms and conditions on her registered nurse license.

In July 2025 the Board of Nursing withdrew the statement of charges on the registered nurse license of Deepak Navin Singh (RN.RN.60397699).

Spokane County 

In May 2025 the Board of Nursing and Beverly Barbara Ballard (LP00059830) entered an agreed order on her licensed practical nurse license. Ballard must contact a monitoring program and, if required, enroll in the monitoring program. In January 2023, Ballard entered into a stipulated order of continuance regarding charges of assault and malicious mischief. In July 2023, Ballard was convicted of reckless driving.

In June 2025 the Board of Nursing indefinitely suspended the registered nurse license of Lara May Jarrett (RN60710737). Prior to petitioning for reinstatement, Jarrett must complete twenty hours of specified continuing education. Jarrett must also contact a monitoring program and, if required, enroll in the monitoring program. In December 2023, Jarrett was observed to exhibit signs of impairment at her place of employment and tested positive for cocaine and cocaine metabolites on a drug screen.

In September 2025 the Board of Nursing suspended the registered nurse and advanced registered nurse practitioner licenses of Paul Brian Means (RN00168881, AP60097094-NP). Means may not petition for reinstatement for a period of at least five years. Prior to petitioning for reinstatement, Means must attend and successfully complete an ethics program. Means must also provide proof of obtaining a competence assessment.

In May 2025 the Board of Nursing reinstated the registered nurse license of Bree Anna Rose (RN60396839) subject to probation. Rose may not practice as a nurse except as part of a pre-approved refresher course and Rose must continue to comply with a monitoring program.

In March 2025 the Board of Nursing withdrew the statement of charges on the licensed practical nurse license of Jennifer Ann Schiff (LPN.LP.61168864).

Thurston County 

In March 2025 the Board of Nursing granted Michael A. Sherbun (RN61490144) a license to practice as a registered nurse with conditions on the license. Sherbun must continue to comply with a monitoring program. In October 2022, Sherbun entered into a deferred prosecution on a charge of driving under the influence.

In July 2025 the Board of Nursing charged registered nurse Janet Lynn Streuli with unprofessional conduct. Charges state Streuli failed to comply with a 2022 stipulation by failing to provide quarterly performance evaluation reports or declarations of unemployment, quarterly personal reports, and proof of completion of required continuing education.

Whatcom 

In April 2025 the Board of Nursing indefinitely suspended the registered nurse license of Laurie D. Dupar (RN00089180). Dupar must pay a fine of $5,000.00, must remove any reference to the term “nurse practitioner” from her promotional materials, and must abstain from promoting herself as a nurse practitioner. Dupar failed to comply with a 2023 board order by continuing to use the term “nurse practitioner”.

Yakima County 

In August 2025 the Board of Nursing entered an order on the registered nurse license of Rebecca Balderas (RN00125380). Balderas must contact a monitoring program and, if required, enroll in the monitoring program. If Balderas is not required to enroll in the monitoring program, her registered nurse license will be placed on probation for at least twelve months subject to terms and conditions. Between approximately 2019 and 2023, Balderas failed to follow facility policy and procedures regarding the wastage of controlled substances.

Out of State 

Arizona

In June 2025 the Board of Nursing indefinitely suspended the privilege to practice in the state of Washington of Monica A. Davis (PTP.LP61650889) as a licensed practical nurse under her Texas multistate credential. Prior to making a written request to lift the suspension on her privilege to practice in Washington, Davis must complete pre-approved continuing education and must meet all requirements to hold a multistate license in any nurse licensure compact state. In May 2024, the Board indefinitely suspended Davis’s Washington single state licensed practical nurse license.

California 

In May 2025 the Board of Nursing reinstated the registered nurse license of Ryan Keith Belloir (RN60682947) provided he meets relevant regulatory requirements for credentialing.

In March 2025 the Board of Nursing indefinitely suspended the registered nurse license of Almeta Bradley (RN61496726). In August 2024, the California Board of Vocational Nursing and Psychiatric Technicians revoked Bradley’s vocational nurse credential for unprofessional conduct to include failure to adequately monitor a patient, failure to document a patient’s condition, and failure to chart patient care in accordance with profession standards.

In July 2025 the Board of Nursing granted Holly Faul (RN61224242) a license to practice as a registered nurse with conditions on the license.  Faul must continue to comply with a monitoring program.

In June 2025 the Board of Nursing reinstated the registered nurse license of Belinda Lovo (RN00165434) subject to conditions. Lovo must continue to comply with a monitoring program.

In June 2025 the Board of Nursing indefinitely suspended the registered nurse license of Arlana I. Ryan (RN60859839). In September 2024, the California Board of Registered Nursing accepted the surrender of Ryan’s registered nurse credential for unprofessional conduct based on diversion of controlled substances.

In September 2025 the Board of Nursing indefinitely suspended the registered nurse license of David Jacob Wareham (RN61191939). Prior to petitioning for reinstatement, Wareham must contact a monitoring program and, if required, enroll in the monitoring program. In May 2024, the California Board of Registered Nursing placed Wareham’s registered nurse credential on probation for thirty-five months based on findings to include obtaining controlled substances and dangerous drugs.

Georgia 

In June 2025 the Board of Nursing indefinitely suspended the Washington single state registered nurse license of Shana Jones Stubbs (RN60954513) and indefinitely suspended    her privilege to practice in the state of Washington as a registered nurse under her Georgia multistate license (Georgia RN176654). Prior to requesting reinstatement of her Washington credential or the privilege to practice under any multistate license, Stubbs must complete thirty-eight hours of pre-approved continuing education. In July 2023, Stubbs used inappropriate technique to restrain a minor patient.   

Idaho 

In May 2025 the Board of Nursing released Jeremy Todd Adams (RN61053308, AP61054516-CRNA) from the terms and conditions on his registered nurse and advanced registered nurse practitioner licenses.

In June 2025 the Board of Nursing indefinitely suspended the registered nurse license of Elizabeth C. Furlong (RN60301901). In December 2024, the California Board of Registered Nursing accepted the surrender of Furlong’s registered nurse credential based on Furlong’s failure to comply with a board order compelling an examination.

In May 2025 the Board of Nursing indefinitely suspended the licensed practical nurse license of Amy K Ross (LP00050636). Ross failed to comply with a 2023 board order by failing to complete a required refresher course and by withdrawing from a monitoring program.

Illinois 

In August 2025 the Board of Nursing released Keandra Deann Jordan (RN61547883) from the terms and conditions on her registered nurse license.

Michigan 

In May 2025 the Board of Nursing suspended the registered nurse and advanced registered nurse practitioner licenses of Fawn Lea Munro (RN61080730, AP61092989-NP). Munro may not petition for reinstatement for a period of at least sixty months. Prior to petitioning for reinstatement, Munro must pay a fine of $5000.00.  In April 2024, Munro was convicted of health care fraud.  

Oregon 

In August 2025 the Board of Nursing suspended the licensed practical nurse license of Kathleen Marie Day (LP00037279). Day may not petition for reinstatement for a period of at least thirty-six months. In December 2024, the Oregon Board of Nursing revoked Day’s license for conduct derogatory to the standards of nursing.

In April 2025 the Board of Nursing granted Caitlin M. Haler (RN61537891) a license to practice as a registered nurse with conditions on the license.  Haler must continue to comply with a monitoring program.

In July 2025 the Board of Nursing indefinitely suspended the registered nurse and advanced registered nurse licenses of Cheryl Lynn Sommer (RN60516833, AP60516834-NP). Prior to petitioning for reinstatement, Sommer must contact a monitoring program and, if required, enter the monitoring program. Sommer failed to comply with a 2024 agreed order by failing to enter a monitoring program.

In September 2025 the Board of Nursing indefinitely suspended the registered nurse license of Steve Cuong Tran (RN60790996). Prior to petitioning for reinstatement, Tran must contact a monitoring program and, if required, enroll in the monitoring program. Tran must also submit a written statement in response to a May 2024 letter of allegation. Tran failed to comply with a 2025 stipulation by failing to submit a written statement in response to a May 2024 letter of allegation and by failing to complete a substance use evaluation.

Texas

In July 2025 the Board of Nursing charged and immediately suspended the registered nurse license of Ashley Michelle King (RN60959741) based on disciplinary action by other state boards of nursing regarding a deferred disposition/deferred adjudication in Texas for unlawful possession of a controlled substance.

Wyoming 

In July 2025 the Board of Nursing released Connie Lee Buck (RN60796613, AP60794627-NP) from the terms and conditions on her registered nurse and advanced registered nurse practitioner licenses.
