OLYMPIA – The Washington State Board of Nursing (WABON) has taken disciplinary action or withdrawn charges against the following healthcare providers in our state.
WABON is committed to protecting public health and safety by regulating the competency and quality of all Licensed Practical Nurses (LPNs), Registered Nurses (RNs), Advanced Registered Nurse Practitioners (ARNPs), and Nurse Technicians (NTECs).

Asotin County
In November 2025 the Board of Nursing indefinitely suspended the registered nurse license of Tracy Lynn Webster (RN00120098). Webster failed to comply with a 2025 agreed order by failing to enter a monitoring program when recommended.
Klickitat County
In December 2025 the Board of Nursing indefinitely suspended the licensed practical nurse license of Carrera Leigh Hobson (LP60200424). Prior to petitioning for reinstatement, Hobson must contact a monitoring program and, if required, enroll in the monitoring program. In January 2018, the Municipal Court of the City of Phoenix, State of Arizona charged Hobson with misdemeanor counts, including driving under the influence of alcohol and hit and run of an attended vehicle. Hobson failed to cooperate with the Board by not submitting a written statement in response to the allegations.
Pierce County
In December 2025 the Board of Nursing charged registered nurse Kimber L. Day (RN00097779) with unprofessional conduct. Charges state that in July 2024, Day exhibited concerning behavior during her shift and was transported to the emergency room. Drug paraphernalia was discovered in Hobson’s possession and Hobson admitted to smoking fentanyl.
In December 2025 the Board of Nursing indefinitely suspended the registered nurse and advanced registered nurse practitioner licenses of David Anthony Gonzalez (RN60977137, AP60977138). Prior to petitioning for reinstatement, Gonzalez must contact a monitoring program and, if required, enroll in the monitoring program. Gonzalez failed to comply with a 2025 stipulation by failing to enter a monitoring program when recommended.
In December 2025 the Board of Nursing indefinitely suspended the registered nurse license of Arthur Leonid Vavrinyuk (RN60644535). Vavrinyuk may petition for reinstatement upon submission of a psychological evaluation report, upon proof of completion of course work in anger management and domestic violence, and completion of an RN refresher course and a jurisprudence examination. In December 2023, the California Board of Registered Nursing revoked Vavrinyuk’s license for unprofessional conduct consisting of pushing his pregnant wife to the ground and holding her down.
In December 2025 the Board of Nursing charged registered nurse Lisa Lynn Zorrozua (RN00166687) with unprofessional conduct. Charges state that in September 2024, Zorrozua was observed to be impaired at work and Zorrozua failed to administer scheduled pain medications to several residents. Zorrozua failed to cooperate with the Board by not submitting a written statement in response to the allegations.
Spokane County
In November 2025 the Board of Nursing indefinitely suspended the licensed practical nurse license of Beverly Barbara Ballard (LP00059830). Ballard failed to comply with a 2025 agreed order by failing to enter a monitoring program when recommended.
In December 2025 the Board of Nursing and Medea R. Karr (RN00158723, AP30006987) agreed to reinstatement of Karr’s registered nurse license and to modification of suspension of Karr’s advanced registered nurse practitioner license. Karr’s registered nurse license is reinstated, provided she meets relevant regulatory requirements for credentialing, and placed on probation for at least 24 months subject to terms and conditions. Karr’s advanced registered nurse practitioner license is suspended for at least 24 months. Prior to petitioning for reinstatement, Karr must complete specified course work in the areas of medical ethics, boundaries and professionalism, medical documentation, and the proper prescribing of controlled substances. Karr practiced below the standard of care in a pain management practice and failed to comply with pain management rules regarding two patients.
Thurston County
In November 2025 the Board of Nursing indefinitely suspended the registered nurse license of Michael Anthony Sherbun (RN61490144). Sherbun failed to comply with a 2025 agreement by failing to comply with a monitoring program. Sherbun tested positive for and admitted to drinking alcohol in April 2025 and withdrew from the monitoring program in May 2025.
In December 2025 the Board of Nursing and Rebecca Suzanne Taylor (RN00171602, AP61201960) entered an agreed order on Taylor’s registered nurse and advanced registered nurse practitioner licenses. Taylor’s registered nurse license is placed on probation for at least 18 months subject to terms and conditions. Taylor’s advanced registered nurse practitioner license is suspended for at least 18 months. Prior to petitioning for reinstatement, Taylor must complete course work in the areas of medical ethics, controlled substance prescribing, and medical record keeping. Between approximately December 2023 and September 2024, Taylor wrote prescriptions for oxycodone in a patient’s name and deleted the prescriptions from the electronic medical record once they were processed.
Whatcom County
In December 2025 the Board of Nursing indefinitely suspended the licensed practical nurse license of Christina M. Petersen (LP00059115). Petersen may petition for reinstatement once she complies with a prior request to submit verification of her compliance with requirements for active nursing practice and continuing education. In December 2024, the Board received a complaint that Petersen had not practiced in over ten years but renewed her license every year. Petersen failed to cooperate with the Board by not submitting verification of her compliance with requirements for active nursing practice and continuing education.
Out of State
California
In December 2025 the Board of Nursing and Elaine Kathryn Haegele (RN60237245, AP60421778) entered an agreed order on Haegele’s registered nurse and advanced registered nurse practitioner licenses. Haegele’s registered nurse license is placed on probation provided she meets all credentialing requirements. Haegele may not practice as a registered nurse except as part of a pre-approved refresher course. Upon successful completion of the refresher course, Haegele’s registered nurse license shall remain on probation for at least two years subject to terms and conditions. Haegele’s advanced registered nurse practitioner license is indefinitely suspended. Prior to petitioning for reinstatement, Haegele must undergo a psychological or psychiatric evaluation.
Idaho
In December 2025 the Board of Nursing charged registered nurse Gretchen Secrest Claypool (RN61057394) with unprofessional conduct. Charges state that in June 2024, Claypool fraudulently completed a visit note report and attached a patient’s electronic signature to the document.
New Jersey
In December 2025 the Board of Nursing reinstated the registered nurse license of Crista Lynn Niles (RN60959508) provided Niles meets relevant regulatory requirements for credentialing.
