OLYMPIA – The Washington State Board of Nursing (WABON) has taken disciplinary action or withdrawn charges against the following healthcare providers in our state.
WABON is committed to protecting public health and safety by regulating the competency and quality of all Licensed Practical Nurses (LPNs), Registered Nurses (RNs), Advanced Registered Nurse Practitioners (ARNPs), and Nurse Technicians (NTECs).

Chelan County
In October 2025 the Board of Nursing released Sarah E. Robins (RN00176672) from the terms and conditions on her registered nurse license.
King County
In November 2025 the Board of Nursing indefinitely suspended the registered nurse license of Tara Jean Kelly (RN60375903). Prior to petitioning for reinstatement, Kelly must meet certain continuing education requirements. Between approximately September 7, 2023, and October 12, 2023, Kelly engaged in practices resulting in controlled substance discrepancies. Kelly failed to cooperate with the Board by not submitting a statement in response to the allegations.
Snohomish County
In November 2025 the Board of Nursing charged registered nurse Elvina F. Kylyukh (RN60809732) with unprofessional conduct. Charges state that Kylyukh failed to comply with a prior stipulation to enroll and participate in a monitoring program. In June 2025, Kylyukh signed a contract with a monitoring program. Kylyukh subsequently missed four check-ins and three test dates. In August 2025, Kylyukh notified the monitoring program she was withdrawing from the program.
Thurston County
In November 2025 the Board of Nursing indefinitely suspended the registered nurse license of Janet Lynn Streuli (RN00112842). Streuli may petition for reinstatement. Streuli failed to comply with a 2022 stipulation by failing to provide quarterly performance evaluation reports or quarterly declarations of unemployment if not working as a nurse. Streuli also failed to send quarterly personal reports after May 2024.
Whatcom County
In November 2025 the Board of Nursing charged licensed practical nurse Christina M. Petersen (LP00059115) with unprofessional conduct. Charges state that in December 2024, the Board received a complaint that Petersen had not practiced nursing in over ten years but renewed her license every year. The license renewal process includes an attestation that all requirements for continuing education and practice hours have been met. Petersen failed to cooperate with the Board by not submitting verification of her compliance with requirements for active nursing practice and continuing education.

Canada
In November 2025 the Board of Nursing charged registered nurse Nicholas Pierre Roger Fudger (RN61355771) with unprofessional conduct. Charges state that in August 2024, the Board received a complaint alleging Fudger engaged in possible drug use. Fudger failed to cooperate with the Board by not providing a written response to the allegations.
Idaho
In November 2025 the Board of Nursing and Cory R. Mczenna (RN60029502) entered an agreed order on modification indefinitely suspending Mczenna’s registered nurse license. Mczenna failed to complete a refresher course required by a 2023 agreed order on reinstatement. Mczenna must provide documentation that she has abstained from the use of any non-prescribed legal or illegal intoxicating, mind altering, or psychoactive substance for at least twelve consecutive months preceding the date of any petition for reinstatement. If reinstated, Mczenna’s license would be subject to probation until she has completed a Board-approved nursing refresher course.
Illinois
In November 2025 the Board of Nursing charged registered nurse Samantha Fox (RN61153085) with unprofessional conduct. Charges state that in August 2024, Fox entered into a monitoring program referral contract with the Board and signed a contract with the monitoring program. Fox was referred back to the Board for failure to comply with the monitoring program. Fox tested positive for alcohol in May 2025 and tested positive for cannabis in July 2025.
Oregon
In October 2025 the Board of Nursing released Kaylan Sheehy (RN61511369) from the terms and conditions on her registered nurse license.
Texas
In November 2025 the Board of Nursing indefinitely suspended the registered nurse license of Ashley Michelle King (RN60959741). Prior to petitioning for reinstatement, King must contact a monitoring program and, if required, enroll in the monitoring program. Between approximately July 31, 2024, and February 6, 2025, King was subject to disciplinary action by other jurisdictions including the Missouri State Board of Nursing, the Illinois Division of Professional Regulation, the Connecticut Department of Public Health Healthcare Quality and Safety Branch, the Texas Board of Nursing, and the Minnesota Board of Nursing.
