OLYMPIA – The Washington State Board of Nursing (WABON) has taken disciplinary action or withdrawn charges against the following healthcare providers in our state.
WABON is committed to protecting public health and safety by regulating the competency and quality of all Licensed Practical Nurses (LPNs), Registered Nurses (RNs), Advanced Registered Nurse Practitioners (ARNPs), and Nurse Technicians (NTECs).

Benton County
In October 2025 the Board of Nursing granted the reactivation of the registered nurse license of Kris Lynn Polk (RN00139143) subject to relevant regulatory requirements for credentialing and with conditions on the license. Polk must contact a monitoring program and, if required, enroll in the monitoring program. In March 2025, the California Board of Registered Nursing adopted Polk’s surrender of her license to practice in California. The California Board action was based on Polk’s failure to comply with a 2024 California Board order entered following a May 2023 conviction for driving under the influence of alcohol.
Pierce County
In October 2025 the Board of Nursing terminated the probation on the licensed practical nurse license of Stephen Wakibi Buteraba (LP60319984).
In October 2025 the Board of Nursing released Cristian Hernandez (RN61493921) from the terms and conditions on Hernandez’s registered nurse license.
In October 2025 the Board of Nursing released Dawn Marie Linquist (RN61089438) from the terms and conditions on Linquist’s registered nurse license.
King County
In October 2025 the Board of Nursing charged registered nurse Emnet Yehuwalashet Bizuneh (RN60235490) with unprofessional conduct. Charges state that in August 2022, Bizuneh allegedly sent type and screen blood specimen vials labeled with a patient’s name to the hospital lab. Both specimens tested as A negative. The patient’s blood type was subsequently determined to be O positive.
In October 2025 the Board of Nursing charged registered nurse Donna Andrea Izenas (RN60385790) with unprofessional conduct. Charges state that Izenas signed a monitoring contract in 2021, repeatedly tested positive for alcohol in violation of the contract, and entered into an Agreed Order with the Board in October 2024 to come into compliance with the monitoring program. In January 2025, Izenas had a positive test showing the presence of ethanol.
Snohomish County
In October 2025 the Board of Nursing released Danielle Marie Regan (RN60979317) from the terms and conditions on Regan’s registered nurse license.
Out of State
Nevada
In October 2025 the Board of Nursing charged and immediately suspended the registered nurse license of Bradley Alan Ermey (RN61012756) pending further disciplinary action. Charges state that between approximately 2022 and July 2025, Ermey engaged in sexual intercourse with a vulnerable adult which resulted in pregnancy.
Oregon
In October 2025 the Board of Nursing terminated the probation on the registered nurse license of Megan Marie Scott (RN61493921).
Utah
In October 2025 the Board of Nursing charged and immediately suspended, pending further disciplinary action, the registered nurse license of Armando Jacob Gameros (RN60735481), and Gameros’ privilege to practice in Washington under his Utah issued multistate credential (9054563-3102). In August 2025, the Wisconsin Board of Nursing revoked Gameros’s privilege to practice in Wisconsin under his multistate credential. The Wisconsin Board based its action on conduct that included diversion of controlled substances, falsifying patient records, and failure to timely cooperate with the Wisconsin Board’s investigation. In October 2025, the Minnesota Board of Nursing revoked Gameros’s credential to practice as a registered nurse in Minnesota for at least five years. The Minnesota Board based its action on conduct that included diverting controlled substances, falsifying patient records, and exhibiting signs of impairment at work.
