OLYMPIA – The Washington State Board of Nursing (WABON) has taken disciplinary action or withdrawn charges against the following healthcare providers in our state.
WABON is committed to protecting public health and safety by regulating the competency and quality of all Licensed Practical Nurses (LPNs), Registered Nurses (RNs), Advanced Registered Nurse Practitioners (ARNPs), and Nurse Technicians (NTECs).

Clallam County
In August 2025 the Board of Nursing withdrew the statement of charges on the registered nurse license of Kathleen M. Humphrey (RN00081564).
King County
In September 2025 the Board of Nursing reinstated the registered nurse license of Renee M. Osborne (RN00167613) subject to relevant regulatory requirements for credentialing and with conditions on the license. Osborne must continue to comply with a monitoring program and complete specified continuing education.
In September 2025 the Board of Nursing entered an order on the registered nurse license of Alicia Kay Sorenson (RN60781540) placing Sorenson’s registered nurse license on probation for three years subject to terms and conditions, including a requirement that Sorenson must contact a monitoring program and, if offered a monitoring contract, enroll in the monitoring program. In October 2023, Sorenson was convicted of driving under the influence.
In September 2025 the Board of Nursing released Jeff Tchai-Fu Su (RN60287904) from the terms and conditions on his registered nurse license.
Klickitat County
In October 2025 the Board of Nursing charged licensed practical nurse Carrera Leigh Hobson (LPN60200424) with unprofessional conduct. Charges state that in June 2024, law enforcement responded to Hobson’s residence as a result of a domestic disturbance and Hobson admitted to drinking alcohol at the time of the incident. Charges also state that in January 2018, Hobson was charged with misdemeanors in Arizona including driving under the influence and hit and run of a vehicle, and a warrant was issued when Hobson failed to appear for her arraignment. Hobson also failed to cooperate with the Board’s investigation by not furnishing a written response to the allegations.
Lincoln County
In September 2025 the Board of Nursing and Daniel O. Sullins (LP60252289) entered an agreed order on his licensed practical nurse license lifting a September 2024 summary suspension and reprimanding his license. Sullins must pay a fine $5,000.00. In November 2023, the Department of Social and Health Services (DSHS), Board of Appeals, issued a Review Decision and Final Order affirming a DSHS determination Sullins had financially exploited a vulnerable adult.
Pierce County
In September 2025 the Board of Nursing granted Lilian Jepkattany Mwetich (LP61645973) a license to practice as a licensed practical nurse with conditions on the license. Mwetich must continue to comply with a monitoring program. In October 2021, Mwetich plead guilty to reckless driving and in August 2023, Mwetich entered into a deferred prosecution agreement on a charge of driving under the influence.
Snohomish County
In October 2025 the Board of Nursing granted Brooke Hilde Babin (AP61311515-NP) a license to practice as an advanced registered nurse practitioner subject to probation for at least nine months with terms and conditions. In November 2024, the Board entered a stipulation placing Babin’s registered nurse credential on probation.
In September 2025 the Board of Nursing indefinitely suspended the registered nurse license of Nancy Jean Eakin (RN00170512). Prior to petitioning for reinstatement, Eakin must re-enter a monitoring program and sign a new monitoring contract. In February 2025, Eakin was referred back to the Board by a monitoring program for failure to follow the recommendations of her treatment program.
In October 2025 the Board of Nursing and Martin Allen Gutierrez (RN61427431) entered an agreed order lifting the September 2025 summary suspension of Gutierrez’s registered nurse license and placing the license on probation for at least twelve months subject to terms and conditions. In November 2024, the Texas Board of Nursing entered an order reprimanding the registered nurse license of Guiterrez and placing his license and any privilege to practice in inactive status. The Texas Board based its action on unprofessional conduct in the practice of nursing including failure to adequately care for a patient or to conform to the minimum standards of acceptable nursing practice.
In September 2025 the Board of Nursing granted Cynthia Riesen (RN61591856) a license to practice as a registered nurse subject to probation for at least two years with conditions on the license. Riesen must also contact a monitoring program and, if required, enroll in the monitoring program. In January 2013, the Ohio Board of Nursing indefinitely suspended Riesen’s registered nurse credential for failure to submit to a mental health examination. In Ohio in February 2013, Riesen was convicted of operating a motor vehicle while impaired, and in March 2013, Riesen was convicted of driving while under suspension. In July 2014, the Ohio Board of Nursing permanently revoked Riesen’s registered nurse credential through a voluntary permanent surrender of license with consent to revocation.
Spokane County
In September 2025 the Board of Nursing and David D. Primm (RN00168794) agreed to a modification of a 2023 agreed order on reinstatement of his registered nurse license. Primm’s license remains on probation subject to terms and conditions. Primm may not practice as a nurse except as part of a pre-approved refresher course and Primm must continue to comply with a monitoring program.
Out of State
Arizona
In September 2025 the Board of Nursing charged advanced registered nurse practitioner Danni Denise Owens (AP61523123-NP) with unprofessional conduct. Charges state that in March 2025, the Arizona Board of Nursing placed the registered nurse and advanced practice nurse credentials of Owens on stayed revocation with conditions based on unprofessional conduct, including but not limited to, false advertising and making false statements to the Arizona Board.
California
In September 2025 the Board of Nursing indefinitely suspended the registered nurse license of David Jacob Wareham (RN61191939). Wareham may petition for reinstatement of his Washington single state license should he no longer hold a multistate credential in any Nurse Licensure Compact state. Prior to petitioning for reinstatement, Wareham must contact a monitoring program and, if required, enroll in the monitoring program. In May 2024, the California Board of Registered Nursing, Department of Consumer Affairs, adopted a February 2024 California Board disciplinary order for stayed revocation placing Wareham’s credential on probation for thirty-five months. The California action was based on findings of incompetence, unprofessional conduct, and obtaining controlled substances and dangerous drugs.
Texas
In September 2025 the Board of Nursing entered an order on the registered nurse and inoperable advanced registered nurse practitioner licenses of Oluleke Anthony Olaiya (RN61327456, AP61333061-NP), indefinitely suspending Olaiya’s registered nurse license. In December 2024, the Texas Board of Nursing entered an order on Olaiya’s advanced practice registered nurse, registered nurse, and vocational nurse licenses.
